Iron-on machine embroidered patch.
2x3" machine embroidered patch with stitched edge and hot melt glue backing.
This patch has glue on the back already and will glue to whatever you want it on, with heat from something like an iron, but I recommend sewing it on if you wear it frequently - or, if you're putting it on smooth fabric.
